Low testosterone: what it means and what to do
In a man, a low testosterone needs confirming: the level changes with the time of day, sleep and your health. If it stays low on two morning tests and there are symptoms, your doctor looks for the cause. Weight, diabetes or medicines are often behind it, and many causes can be corrected.
What low testosterone means
In a man, low testosterone with symptoms is called hypogonadism. The diagnosis needs two fasting morning tests, both low, and symptoms that fit.
A single low result without symptoms often means nothing: an afternoon test, a sleepless night or flu is enough to cause it.
In women, testosterone is normally low. It falls slowly with age, and the pill lowers the free part. In a woman, a low result is rarely a problem on its own.
What causes low testosterone
In men the most common come first, and they can often be corrected.
- Excess weight and belly fat: Fat around the belly lowers testosterone. Losing weight often brings it back up.
- Diabetes and insulin resistance: They often go together with low testosterone.
- Sleep and sleep apnoea: People who sleep little or snore with pauses in their breathing often have lower levels.
- An illness or acute stress: After an illness or a hard period it drops, then usually comes back.
- Medicines and substances: Opioids, high-dose steroids such as prednisone, anabolic steroids even if stopped long ago, a lot of alcohol. Do not stop medicines on your own.
- The pituitary gland: A high prolactin or a pituitary problem can weaken the signal that reaches the testicles.
- The testicles: Less often the problem is in the testicles: a genetic condition such as Klinefelter syndrome, an injury, chemotherapy.
Symptoms of low testosterone
In men, the most typical symptoms are sexual. For example:
- Lower sex drive
- Erection problems, and fewer morning erections
- Tiredness, low mood, poorer concentration
- Less strength and muscle, more fat on the belly
- Less body hair, hot flushes if the drop is large
These symptoms have many other causes too. That is why testosterone is read together with an examination.
Low testosterone and age
Testosterone falls a little over the years, but in a healthy man the fall is slow. Much of the drop seen with age comes from weight, diabetes and other illnesses.
Testosterone therapy is decided with your doctor, and only with a real diagnosis. It is not for men who want children soon: testosterone taken from outside reduces sperm production.
Which tests to read alongside it
These tests help show where it comes from.
How to raise your testosterone
It depends on the cause. Where to start:
- First, repeat the test in the morning, fasting, when you are well.
- If you carry extra weight, losing some is often what helps most.
- Move every day, with some strength work, and sleep enough. If you snore with pauses in your breathing, tell your doctor.
- Drink less alcohol.
- Before taking supplements or products for testosterone, ask your doctor.
Do not use testosterone or steroids without a prescription: they can switch off your own production and reduce fertility.
Low testosterone: when to worry
Talk to your doctor if:
- Your testosterone is low on two morning tests.
- You have lower sex drive, erection problems or fewer morning erections.
- You and your partner have been trying for a baby for more than a year.
- You have headaches that do not go away, changes in your vision or milk from a nipple: see your doctor soon.
- You have a sudden, very severe headache with problems seeing: call 112 or go to the emergency department.
